Thriving Families NSW provides targeted support to meet the needs of vulnerable young parents aged 25 years and under, and their children (aged 0 – 5 years).
The aim of Thriving Families NSW is to enhance the wellbeing of vulnerable families and decrease the likelihood of them reaching crisis point. Thriving Families NSW does this by ensuring young parents have access to age-appropriate, strength-based wraparound services, which meet the needs of the entire family.
Thriving Families NSW draws on services from across NSW Government and non-government organisations to deliver a coordinated and multi-agency service solution that will empower young families to thrive.
The Thriving Families NSW team
A dedicated Key Worker provides a single point of contact, supporting families with access to the right service at the right time and the support they need. This includes their health needs, as well as other areas like housing, education, employment and their child/ children’s development.
Facilitated Family Meetings connect the family’s support network to work in partnership with services to set and achieve their identified goals. Based on the needs identified by the family in these meetings, they will have access to a number of people within the Thriving Families NSW team.
A Multidisciplinary Youth Health Team provides outreach support, including antenatal and postnatal health services. An Education Coordinator can assist young parents to continue or resume study. A cross-agency Coordinator acts as a central point of contact to facilitate and coordinate access to solutions and services, to escalate issues, and to monitor and report on outcomes of the initiative.
Thriving Families NSW pilot site
Their Futures Matter is committed to ongoing monitoring and evaluation of the delivery of outcomes for families participating in the Thriving Families NSW pilot, which was launched in Western Sydney in October 2018.
